All Professional users with Glooko Enterprise accounts have access to the Population Health tool, allowing you to create advanced filters on your patient population, target at-risk patients, and to track statistics post-intervention.
Accessing Population Health
To access the Population Health platform and begin targeting your at-risk population:
- Select the circle with the account initials in the top right
- Choose Population Health
Utilizing Population Health
There are two tabs available within the Population Health platform: At-Risk and Pre/Post.
At-Risk Tab
The At-Risk tab provides clinicians with advanced filters that can be used to further segment their patient population, as well as the ability to create custom tags based on those filters, in order to quickly identify at-risk patients from the Population Tracker.
The following filters can be applied to your patient population from the At-Risk tab.
Demographics
- Diabetes Type
- Type 1
- Type 2
- LADA
- Gestational
- Pre-Diabetes
- Other
- Age
- Greater than X
- Less than X
- In Between X
Beyond A1C Measurements
Before filtering on the beyond A1C measurements, select a time frame. The available time frames are:
- 7 days
- 14 days
- 30 days
- 90 days
- 180 days
Once you’ve selected the time frame, you may utilize these additional Beyond A1C measurements:
- SMBG Check Rate
- Greater Than X times a day
- Less Than X times a day
- In Between X times a day
- CGM Active Time
- Greater Than X%
- Less than X%
- In Between X%
- Average Glucose Level
- Greater Than X mg/dL or mmol/L
- Less Than X mg/dL or mmol/L
- In between X mg/dL or mmol/L
- Percentage of Glucose Level Readings
- X% Greater Than X mg/dL or mmol/L
- X% Less Than X mg/dL or mmol/L
- X% In Between X mg/dL or mmol/L and X mg/dL or mmol/L
- Absolute Glucose Level Readings
- Any readings greater than or equal to X mg/dL or mmol/L
- Any readings less than or equal to X mg/dL or mmol/L

Once you’ve selected your filters, your At-Risk tab will display a summary of the filters near the top of the page, along with the option to Save Custom Cohort utilizing these filters, as well as the ability to view matching patients within the population tracker by selecting View Patients in Population Tracker.
You may also select View All Cohorts to see both saved custom cohorts and default cohorts.
Saving the filters as a custom cohort allows you to tag patients that meet your defined criteria. To learn more about creating and utilizing custom cohorts, see our article Population Health: Custom Cohorts.
The bottom half of the page will display statistics related to the population captured by your filters, including the number of patients matching the criteria, along with information about their devices. You will also see a table populated with Glucose Level Statistics based on the cohort’s data.
You will also have the option to export the data to a CSV by selecting the Export Table to CSV just below the Glucose Level Statistics table. The generated CSV will be password protected. A pop up will open providing you with the randomly generated password for access to your CSV.
Pre/Post Tab
The Pre/Post tab allows you to comparatively visualize data from before and after certain interventions. The available interventions are: Started on Glooko and Last Clinic Visit.
You can further adjust the Pre/Post visualizations by adjusting the Time Increment, Time Before Intervention and Time After Intervention. You may also further filter the pre/post comparison by the following criteria:
- Diabetes Type
- Age
- Remote Monitoring
- Devices
The charts available on the Pre/Post tab include:
Population Average Glucose Level Over Time
Population Count
Glucose Level Changes (mg/dL or mmol/L)
Glucose Meter Use
Similar to the At-Risk Tab, you may also export the data from the Pre/Post tab to CSV by selecting the Export Table to CSV option found at the bottom of the page.
