You can quickly and easily create a new patient account from the Patient List in your Population Tracker.
TIP: Entering the patient's email address allows you to invite the patient to use Glooko at home. You also have the option to invite the patient to join Glooko after the account is created.
To create a patient account
- Click Create Patient Account at the top-right of your Patient List screen.
- Enter the following information:
- First Name
- Last Name
- Birthdate
- Medical Record Number (if required by your clinic)
- Email Address (if required by your clinic)
NOTE: If your patient has already created a Glooko account using the entered email address, you will not be able to create a new account using that email address. To have the patient's personal Glooko account in your clinic list, the patient will need to link to your clinic by adding your clinic's ProConnect code in their personal account: How do I ProConnect with my healthcare provider? - Type of Diabetes (if required by your clinic)
- Phone Number (if required by your clinic)
NOTE: If you are creating an account outside of the U.S., you will also be required to confirm the patient's consent to share data.
- Click Create.
