The Population Tracker now features a new Flags column in your Patient List for health systems utilizing the Population Health feature. Whenever a patient satisfies certain at-risk criteria, a new flag will display within this column. Currently, the flags are auto-updated on a daily basis at 8am PST/PDT.
There are 5 default flags that display in blue in the patient list:
- Dropout Risk
- Hypo Rate
- Hyper Rate
- Marked High
- Marked Low
What is the Marked High flag?
A Marked High flag appears when one or more of the patient's glucose values within the past 7 days fall above a specified glucose amount. Glooko's default value for the glucose threshold is 250 mg/dL.
The glucose threshold may be modified from the default value to the desired value via the account Settings in Population Flag Settings. Flag thresholds adjusted in the Population Flag settings will apply to the entire population. You may revert to the default Glooko value for these flags by selecting Restore Default.
In addition, the threshold can be modified for an individual patient, via the patient's settings on their Profile under Patient Flag Settings. You may also revert an individual patient's to the thresholds established in account settings by selecting Restore to Population Settings.
Creating Custom-Defined Flags
In addition to the default Glooko flags, healthcare providers can also create custom flags based on their own at-risk criteria. Once created, these flags will auto-update on a daily basis on the Population Tracker patient list.
